If you have encountered an incident that resulted in malfunction or damage to a device, ensure that you fill this form before submitting a ticket. A request without an incident report will not be attended to.


In the form, you'll be asked to provide information leading to the incident, including the device details.

In the case of damaged or lost device(s), once an incident report is submitted, we will reach out to your manager for approval to replace the device.
